Entrance to four of the five U.S. service academies requires nomination by a Member of Congress or the Vice President.  The Coast Guard Academy does not require a nomination.  Students who reside in the 11th Congressional District of Ohio may obtain more information and nomination application forms at http://fudge.house.gov/military-academy-nominations/. After nominations are made, offers of appointment are generally sent out by May.
